If you have purchased e-commerce subscription from Dun & Bradstreet Limited and would like to cancel it, please complete this cancellation request form. Once submitted, your subscription will be cancelled as set out below. Please note that submitting this form does not automatically cancel your subscription.

 By ticking this box and submitting this form, you agree that you are an authorised representative of the Company listed on the form and that you have the authority to cancel the aforementioned service. You recognise that once you submit this form the cancellation may take up to 48 hours to complete. Should you wish to cancel the service in advance of automatic renewal, you must submit this form before the set number of days mentioned in the terms of use for the aforementioned service. Where you are unsure of the number of days' notice required to effect cancellation without renewal or to determine when you will lose access to the service, we would recommend that you review the terms of use associated with the service on your Customer Portal. Where you submit this form after your Company’s renewal date, the cancellation will only apply to the next contract term (commencing after the current term).
